What is Sensitive NPI (Non Public Information)?
 
Sensitive Non Public Information includes the following items whether stored in electronic or printed format.
 
1. Financial Information
 
2. Credit Card Information
  • Number – in part or whole
  • Expiration Date
  • Cardholder Name
  • Cardholder Address
3. Identification Numbers
  • Social Security Numbers
  • Social Insurance Number
  • Business Identification Number
  • Employer Identification Number
  • Tax ID Numbers
4. Payroll information, including:
  • Paychecks
  • Paystubs
  • Pay rates
5. Medical/Dental Insurance Numbers
  • Doctors Names and claims
  • Insurance claims
  • Prescriptions
  • Any related personal information
6. Phone Numbers
 
7. Other Personal Information belonging to Customers, Employees and Contractors, examples would include:
  • Date of Birth
  • Address
  • Phone Numbers
  • Maiden Name
  • Names
  • Customer Number
  • Invoice Number
  • Driver’s License Numbers
8. Vendor Information
 
9. Supplier Information
 
10. Company – Proprietary and or Confidential Information
